(ASCEND) TNT Modem POST Failure...

I have a TNT running 7.0.4 configured as follows:

Shelf 1 ( master ):
    { shelf-1 slot-1 0 }       UP       t3-card
    { shelf-1 slot-2 0 }       UP       hdlc2-card
    { shelf-1 slot-3 0 }       UP       48modem-56k-card
    { shelf-1 slot-4 }         OCCUPIED
    { shelf-1 slot-5 0 }       UP       48modem-56k-card
    { shelf-1 slot-6 }         OCCUPIED
    { shelf-1 slot-7 0 }       UP       48modem-56k-card
    { shelf-1 slot-8 }         OCCUPIED
    { shelf-1 slot-9 0 }       UP       48modem-56k-card
    { shelf-1 slot-10 }        OCCUPIED
    { shelf-1 slot-11 0 }      UP       8t1-card
    { shelf-1 slot-12 0 }      UP       4/1ether-card
    { shelf-1 slot-13 0 }      UP       48modem-56k-card
    { shelf-1 slot-14 }        OCCUPIED
    { shelf-1 slot-15 0 }      UP       csmx-card
    { shelf-1 slot-16 0 }      UP       csmx-card
    { shelf-2 slot-3 0 }       POST     48modem-56k-card
    { shelf-2 slot-4 }         OCCUPIED
    { shelf-2 controller 0 }   UP       shelf-controller

I am getting the following errors:

LOG error, Shelf 1, Controller, Time: 19:15:35--
  Slot 2/3, timeout failure code=0 during POST

LOG error, Shelf 1, Controller, Time: 19:18:31--
  Slot 2/3, timeout failure code=0 during POST

LOG error, Shelf 1, Controller, Time: 19:20:33--
  Slot 2/3, timeout failure code=0 during POST

LOG error, Shelf 1, Controller, Time: 19:22:35--
  Slot 2/3, timeout failure code=0 during POST

I also see the following:  SAR: Failed internal loopback 

Does anyone have a clue?  I have already tried different slots, and a new
modem card.  Nothing seems to solve the problem.

Any help would be greatly appreciated.
