Try this:

isdnuser  Password=pass, NAS-Port-Type=Sync
          ... other parameters...

modemuser  Password=pass, NAS-Port-Type=Async
          ... other parameters...

eitherway  Password=pass
          ... other parameters...


Parameters you put on the _same_ line as the username are used to
gate the profile.
