That means the modem has been marked as bad for one reason or another and 
needs to be reset.  That would be swell and all if ascend could reset just 
one modem but that is something they cannot do at this time.  The modem 
will stay on the 195 list until all other modems on a modem card are unused 
and only then will the modem be removed from the 195 list.  The long steady 
tone comes from the calls being routed to the HDLC (slot 9) card on the 
max...you can see this in the 'show users' command.

We have found that the only way to eliminate this is to downgrade the 
firmware to 7.0.4 which is the latest version of the firmware that does not 
have a 195 list therefore the problem is not possible.

The last word I have had from Ascend is that the issue is being passed on 
to engineers so I can only hope there is a fix for this on the way.
