It's possible to handle this via radius.  I know of an organization here
in Houston that offers dialup to it's members.  They had a problem with
multiple people sharing their accounts, so they wrote a little feature
into Merit Radius that would drop all users using a specific account name
if some tried to use that account when it was already in use.
