As you're apparently a small ISP, forget about #s 1-5 and concentrate
on 5. Several CLECs colo DLAMs with the ILEC and thereby provide DSL
to ISPs for resale. They will answer #2 for you, depending on the
DSLAMs they use.
To name a few:
Covad http://www.covad.com/
Northpoint http://www.northpoint.net/
Rhythms http://www.rhythms.com/
Your local ILEC may also wholesale DSL (probably ADSL) too.
There are also other CLECs and ISPs who act as aggregators, bringing
you DSL from other providers. I can think of a few offhand,
Flashcom http;//www.flashcom.com/
ICG http://www.icgcomm.com/
Zyan http://www.zyan.com/
You will probably find that the additional layer of the aggregator
will temp you to refer to them as an aggravator. Of course, CLECs are
phone companies, and therefore a pain to deal with too. ;-)
You may also be able to find an ISP that already has a connection to
a CLEC or three and will wholesale to you. We do this on a small
scale in certain regions, but this isn't a sales email.
I'm not necessarily recommending any of the above companies. In fact,
I know that some (most?) of them are boneheaded.
